After being in practice for nearly four decades, we have made the difficult decision to retire as of January 31st 2025.
It has been an honor and a privilege to take care of you during this time.
We advise you contact your insurance company for names of primary care doctors accepting new patients.
Effective immediately we have retained “Desert River Solutions” to be the custodian for your medical records. Your records will be securely stored and transmitted using HIPAA compliant technology meeting all state and federal laws. Your records will be available to request for 7 years after the practice closes. There is a fee to release your medical records, but it is reasonable and is capped by the Texas State Board of Medicine.
REQUESTING YOUR MEDICAL RECORD IS A SIMPLE PROCESS, HERE ARE THE STEPS…
1. Visit the Desert River Solutions website at www.DesertRiverSolutions.com/requests
2. Go to “Request Your Records Here”
3. Create an account, and remember your unique username and password, so you can easily log in to check on the status of your request
4. Log in and complete all sections of the Authorization for Release of Medical Records Form. Make sure to include the doctor's name and practice name
5. Select how you would like to receive your electronic medical record, in a downloadable link via email or on an encrypted thumb drive by mail.
6. Next, enter the information for where you would like to have your record sent. You can have your medical record sent directly to you or enter your new physician's information to have it sent to them.
7. Review the form for any errors, then click the "Submit" button; and,
8. Finally, submit a secure credit card payment of $25 to complete your medical record request.
You're all set! Once you've finished the above steps, your records will be sent within 15 business days. To track the progress of your request, simply log into your account and view the dashboard.
For the quickest and most convenient processing of records release requests, we highly recommend that all patients submit a records release form electronically, using the above steps.
If needed, you have the option to print and mail a paper medical record release form or email the completed PDF using the steps below. Be sure to include your $25 payment by mail.
REQUESTING YOUR MEDICAL RECORD BY MAIL, HERE ARE THE STEPS…
1. Visit the Desert River Solutions website at www.DesertRiverSolutions.com/requests;
2. From the Patient Records Request page, scroll all the way down to the Frequently Asked Questions section and click on “Can I submit a paper medical release form?”;
3. Next, click on the Download A Blank Release Form (PFD) Here link
Important! Use the record release form provided by Desert River Solutions. This is necessary for ensuring the accuracy and legality of the documentation;
4. Complete all sections of the Authorization for Release of Medical Records PDF.
5. Select how you would like to receive your electronic medical record, in a downloadable link via email or on an encrypted thumb drive by mail;
6. Write the information for where you would like your record sent. You can choose to have your medical record sent to you or enter your new physician's information to have it sent directly to them;
7. Sign and date the form, be sure to review for any errors and check for legibility before sending it; and,
8. Finally, mail your $25 payment with the completed medical request form to:

If you need a printed copy of your medical record but don't have access to a computer or printer, please contact your preferred printing service or office supply company.
USB thumb drives can be plugged into your computer's USB port. They are commonly used for file storage because of their portability, speed, and ease of use and are widely supported for transferring files between computers. However, some organizations may have policies prohibiting the use of USB drives carried in from outside of their organization. Once you transfer a copy of your medical record to your computer, you can attach it to an email, upload it, or print it.
Here are six simple steps to use a USB thumb drive…
1. Insert the USB thumb drive into an available USB port on your computer.
2. Wait for your computer to recognize the thumb drive.
3. Open File Explorer (Windows) or Finder (Mac) to locate the USB thumb drive (only if your thumb drive is encrypted, enter the unique password included in the envelope from Desert River Solutions).
4. Drag and drop the files you want to transfer onto the USB thumb drive.
5. Safely eject the USB thumb drive from your computer.
6. When not in use, store the USB thumb drive (and password if encrypted) in a safe place.
